Karin Parotta was a modeling agent who helped Lois Griffin jumpstart her modeling career. She calls Lois "sugar". As the success of Lois' career grew, Karin pressured her to take diet pills. She was a heavy smoker and, as a result, had a large stoma in her neck. This stoma was a turn-on to Quagmire, who asks "Hey, do you ever get freaky with that thing?". Karin could also dispense Pez candy from it.
